Maisha, a 4.5
years old girl comes from Pakistan. In September 2014, has the first symptoms
"fever and cough". In Aga Khan University Hospital (Pakistan)
hospital’s diagnosis was "acute B lymphoblastic leukemia". A course
of "VP" chemotherapy was given, and the bone marrow morphology
reexamination revealed a remission of the disease, and 8 courses of regular
chemotherapy were followed, and the bone marrow morphology and immunity residue
were not monitored during this period.
In May 2017, she had a fever again. 3.16% was malignant blasts B
lymphocyte. The result of blood routine and bone marrow morphology in the local
hospital showed a relapse of the disease.
On August 18, 2017, after the patient compared the BMT successful
rate and expenditure of many medical centers globally, she came to our hospital
for the first time for further treatment. Bone marrow biopsy test showed she is
not in complete remission, it means her condition is not ready for blood and
bone marrow transplantation.
After a multi-discipline consultation meeting, our
hematologists suggest her to take part in the CAR-T clinical trial before BMT.
Cause CAR-T have a big chance to make her into complete remission which is the
best condition for BMT. In our 300 cases of CAR-T, the complete remission rate
is about 90%.
On Sept. 1st CD19-CART cells infusion was done as cellular immune
therapy, three reviews followed all showed that she got complete remission, and
there were no malignant blasts cells were seen in the immune residual. This
lays the foundation for the next step-Blood and Bone Marrow Transplantation.
The China Marrow Donor
Program(CMDP) as First time Donated Hematopoietic Stem Cells to Pakistani
Patient
The CMDP found an unrelated female donor whose
HLA matching site is 10/10 with Maisha. On December 5, 2017 is a miracle date
for Maisha. Allogeneic hematopoietic stem cell transplantation was carried on
for her. Leucocyte graft survival at +15 days, platelet
graft survival at +6 days. Appeared infectious diarrhea at -3 days, the
stool returned to normal after positive local anti - inflammatory treatment. On
December 22, she finally out of air laminar flow room. She is in disease free
condition right now
Pakistani Embassy Deputy Came to
Visit Maisha’s Family
Mr. Ahmad Bilal Malik, the Pakistani embassy deputy were invited
by our international center to visit Maisha at the hospital. He went to
Maisha’s room with gift and wish her could get well soon. “The environment,
